In Dungeons & Dragons, a Blood Goddess typically refers to a powerful and malevolent deity associated with blood, sacrifice, violence, and domination. While not part of the core pantheon in every edition, this concept appears across campaign settings, particularly in darker, more brutal narratives where blood magic, war, and corruption take center stage. One prominent example is Zehir, the serpent god of darkness and poison, whose worship sometimes overlaps with blood rites. However, a more direct representation is Kiaransalee, a drow goddess of vengeance and undeath, or Lolth, the Spider Queen, who demands blood sacrifices and thrives on the suffering of her followers. Still, in some settings, homebrew or third-party content expands on the idea of a Blood Goddess in more literal terms.

One of the most recognized blood-themed deities is Loviatar, the Maiden of Pain, from the Forgotten Realms. Although not explicitly titled the Blood Goddess, her dominion over pain, suffering, and ritual flagellation often involves bloodletting and sacrifice, making her a central figure in campaigns focused on torment and fanaticism. In darker corners of the multiverse, evil cults may worship obscure or forgotten blood goddesses invented by the Dungeon Master—divine beings who grant power in exchange for ritual blood offerings. These deities often favor clerics, warlocks, and paladins who are willing to wield divine power through violence, punishment, and gruesome rites.

In some interpretations, especially in settings inspired by Gothic or horror themes like Ravenloft, a Blood Goddess might embody both temptation and terror. She can appear as a figure who offers immortality through vampirism or who manipulates blood as both a symbol and a weapon. In such settings, her followers perform grotesque ceremonies, manipulate life essence, or even use blood as a magical conduit. The Blood Goddess trope functions well as a symbol of the raw, primal forces of life and death, making her a compelling figure for morally gray or outright villainous characters. Her power lies not only in divine might but in the fear and awe she inspires through the visceral act of blood sacrifice.
