While D&D does not have an official stat block for an Akaname, it is often adapted as a creature from the broader Yokai mythology in settings or campaigns that draw on Eastern influences.

The Akaname, as interpreted in D&D, is a humanoid monster with a grotesque appearance. Its body is typically slimy or covered in a film of dirt, and its mouth is often large and exaggerated, resembling that of a toad or frog, perfect for licking surfaces. Its skin can appear sickly or decayed, and it tends to emanate a foul, pungent odor. The creature's primary activity is to seek out unclean or neglected areas, feeding off the grime, mold, and filth. In folklore, the Akaname serves as a warning against poor hygiene, as it is said that its presence is a consequence of carelessness in keeping one’s surroundings tidy.
