The kuo-toa in Dungeons & Dragons are a race of fish-like humanoids originally created for the Underdark setting, notable for their alien appearance and bizarre culture. First introduced in the early editions of the game, kuo-toa are amphibious creatures that dwell in the lightless depths of the world, often along the shores of underground lakes and rivers. Their slippery, scaly bodies, bulbous eyes, and webbed hands and feet give them a distinctly piscine look, often compared to deep-sea fish or grotesque parodies of humanoid life.

Once a seafaring coastal people, the kuo-toa were driven underground long ago by surface-dwelling races. Cut off from the sun and left to fester in the dark, they developed an insular and superstitious society, becoming deeply insane over generations due to both their isolation and the influence of malevolent extraplanar forces. This madness plays a central role in their lore, as kuo-toa possess a unique psychic ability: through collective belief, they can bring gods into being. These entities, known as demigods or "godlings," are usually bizarre and grotesque, reflecting the twisted desires and fears of the kuo-toa who imagine them. The most infamous of these deities is Blibdoolpoolp, the Sea Mother, depicted as a nude female human with the head and claws of a lobster, who demands sacrifice and commands absolute devotion.

Kuo-toa society is tribal and theocratic, governed by priests and religious leaders whose authority comes from their claimed connection to these godlings. Their settlements are often labyrinthine warrens of slime-covered tunnels, dotted with shrines and grotesque idols. Though not especially powerful individually, kuo-toa are dangerous in groups due to their fanatical zeal, cunning tactics, and strange magical abilities, such as nets that tangle foes and sticky slime that can disable weapons. Their lore is a fusion of cosmic horror and dark fantasy, making them eerie and unsettling foes in any campaign.
