A Small Deep One in Dungeons & Dragons is a twisted, amphibious humanoid born from the unholy union of mortals and entities of the deep sea—typically Great Old Ones or other aberrant oceanic powers. These creatures are often used in campaigns with a Lovecraftian flavor, particularly in settings involving eldritch horrors, sunken ruins, or cults devoted to alien gods. Though not part of the official core rules, Small Deep Ones commonly appear in third-party supplements or custom content inspired by H. P. Lovecraft’s mythos, especially the infamous Deep Ones of The Shadow over Innsmouth.
Physically, a Small Deep One is a hunched, slimy creature with mottled gray-green skin, webbed hands and feet, bulging lidless eyes, and gills fluttering on either side of its neck. While some retain remnants of their mortal ancestry, most have fully transitioned into alien forms. Their size category as "Small" means they’re typically 3 to 4 feet tall, possibly juvenile or runt versions of their larger kin. Despite their stature, they remain unnerving in both appearance and behavior.
Mechanically, Small Deep Ones are often presented as minions or low-level cult agents. They may be used to harass adventurers near coastal villages, guard forbidden temples, or serve as sacrifices in oceanic rituals. Their stat blocks usually include amphibious movement, resistance to cold, and an unsettling gaze or croaking chant that causes fear or madness. Some have innate spellcasting tied to the depths, such as dissonant whispers, command, or cause fear. Their loyalty to alien sea gods like Dagon or Tharizdun makes them fearless and fanatical.
Narratively, they offer an entry point into deeper cosmic horror plots. A campaign might begin with sightings of these small, fishlike figures lurking near a shoreline, leading to investigations that spiral into revelations about ancient pacts, interbreeding, and a coming apocalypse from beneath the waves.
