Sorrowsworn are terrifying creatures in Dungeons & Dragons, often associated with the Shadowfell, a plane of gloom and despair. They are physical manifestations of intense negative emotions such as anger, fear, hunger, and loneliness. These beings are shaped by the oppressive atmosphere of the Shadowfell, where emotions can warp creatures into monstrous forms.

In Mordenkainen’s Tome of Foes, Sorrowsworn are depicted as twisted horrors, each reflecting a different type of suffering. Some, like the Wretched, are small, scuttling creatures driven by desperate hunger, while others, such as the Angry or the Lost, embody rage or hopeless wandering. Their monstrous traits reflect their emotional origins—clawed hands, distorted bodies, and eerie, shadowy forms.

Sorrowsworn are drawn to strong emotions, feeding off the suffering of others. They often serve as tormentors or mindless predators, attacking those who display the feelings they embody. In combat, their abilities are designed to punish players for displaying weakness. For example, the Lonely grabs and drains life from isolated victims, while the Angry becomes more powerful when it takes damage.

These creatures make excellent horror-themed encounters, emphasizing despair and psychological dread. A Sorrowsworn encounter can be more than just a battle—it can highlight the Shadowfell’s oppressive nature, pushing characters toward fear, sorrow, or rage. Their presence can also serve as a warning of a location’s corruption, hinting at a larger force manipulating emotions.

Though typically bound to the Shadowfell, Sorrowsworn can appear elsewhere, especially where suffering is strong. A powerful necromancer, fiend, or curse might summon them, creating new dangers beyond the Shadowfell. Their connection to deep emotional torment makes them not just monsters but living embodiments of despair, reinforcing the themes of darkness and sorrow in a campaign.
