In Dungeons & Dragons, a Blood Spirit is not a standard creature or concept officially published across core rulebooks like the Player's Handbook, Monster Manual, or Dungeon Master's Guide. However, the term “Blood Spirit” has appeared in various homebrew content, third-party supplements, and campaign settings, where it takes on different meanings depending on the narrative and magical framework of the world.

In general terms, a Blood Spirit is often portrayed as a supernatural entity that is intrinsically tied to blood—whether it be the blood of a particular lineage, the blood spilled during combat, or ancient blood magic. These spirits are usually manifestations of primal energy, ancestral will, or raw emotion, and they are frequently summoned or invoked through rites involving blood sacrifices or deep emotional trauma. They may be guardians of family lines, avengers of murdered kin, or spirits born from wars and massacres, feeding on the residual energy of spilled life-force.

In campaign settings that include blood magic as a discipline, Blood Spirits might serve as familiars, patrons, or sources of power for warlocks, sorcerers, and shamans. A character might gain access to unique abilities by forming a bond with a Blood Spirit, such as enhancing their spellcasting, gaining resistance to necrotic damage, or draining life from foes. Thematically, these spirits often blur the line between the divine and the profane—while some are revered as ancestors or protectors, others are feared as curses or malevolent echoes of violence.

In a roleplaying context, Blood Spirits can add depth to a character’s backstory or the campaign's lore. For instance, a barbarian whose rage is fueled by a Blood Spirit ancestor might see visions of their forebears in battle. Alternatively, a haunted village might be plagued by a Blood Spirit seeking vengeance for an ancient betrayal. The concept is flexible, darkly magical, and often deeply personal.
